This single page design is one page out of my monthly wellness pack.

- Treatments List: List of substances. Track how much of any substance you consume. Spaces also provide enough room for other items like daily medications, foods, or habits you would also like to track. Want to cut down on your alcohol and track your caffeine consumption? Great, for every pre-decided amount (like a mug of coffee) put a dash down beside it in one of the columns. I use them for morning, afternoon, and night to estimate when the most of something gets consumed or used. Had a bath? Write it here too - it's a treatments list.
